Industrial User Survey
Form must be completed by all businesses in the City every 5 years. The next time to be done is in 2007. Form must be filled in and returned to Wastewater Treatment Plant.

Baseline Monitoring Report
Form to be filled out by all Categorical Users and Significant Industrial Users. To find out if your company fits either one of these definitions, contact the Wastewater Treatment Plant.

All forms may be printed from your home or office and returned to the appropriate City office via mail, fax, or walk-in.
